Uploading Your Student’s Physical

Physical Information

All participants need to have a current physical on file with the athletic department before they can participate. Physicals are good for a full calendar year. Physicals can also be emailed to pearson_christy@svvsd.org or brought into the athletics office.

Walk-In sports physical can be done at all UCHealth Urgent Cares in Longmont. The cost is $30.

CHSAA Approved Physical Form

All athletes must use this CHSAA-approved physical form to be cleared by their doctor to participate. Only Page 4 is required to be turned in to the school. Make sure all information is filled out on Page 4 before uploading.
